i think i have seen them as fantasy items for sale, it is unusual to use a silber 990 mark, normal coins like this are mostly continental silver like 800 or 835
i can be wrong but ther is something odd about it

I agree, I've been collecting coin and medallions of TR for couple of years and I have never seen one like this before, however there are thousands of fantasy items out there. I believe this is one of them.
