The skull's also had a back part, so that it was a complete skull!

Are you sure it's a medal and not a watch fob? It's just that there's no scale to compare it. It could possibly be a university/sorority type of arms, these were very popular in the U.S. at one time. Similarly, it may have masonic or knights templar type links, but it certainly isn't a military related piece IMO.
