This one technically doesn't count (it is a crusher), but the entire front of the hat is on the same plane--the leather visor has no angle at all in relation to the band:

Thought I'd make a small contribution to this really interesting thread, with my example. A somewhat beaten up NCO visor but it has a fantastic shape, and so much character.
It one of my favourite pieces for it's pure 'been there' ness.
It does not address the "same plane", however, this early Clemens Wagner sheet advertises the different types of visors that could be ordered:
“Show me the regulation, and I’ll show you the exception.”
GJ EM/NCO visor dated 1939:
“Show me the regulation, and I’ll show you the exception.”
Mid- to late-war Drechsler Artillery Officer:
“Show me the regulation, and I’ll show you the exception.”
LW Sonderfuehrer:
“Show me the regulation, and I’ll show you the exception.”
This has been fascinating to research this thread. I thought I would just add my pioneer NCO visor for reference. though I'm sure with the great information posted here it's probably not needed.
Another nice specimen that fits into this category.
