What does the ARZT mean?
The cap has a unusual bullion eagle, theater made?

Ass.-Arzt = Assistant physician
I am no specialist for insignia but even if this is a rarer variety of an eagle there were such things - depending on who (which firm/individual) embroidered it.

Never seen this insignia before in my life but even so, I'd expect a theater to have even worse quality than that as the people watching wouldn't have to come up close to the hat, right?
