It is a medallion of office for the masons it was worn by the tylers who were guards or keepers to turn away anyone who didnt have business with the masons when meetings were in progress.

Im sorry i couldnt tell you a age without knowing what the stamp is in the back of it. I cant make it out in the photos, i dont know about rariety or if the masons still use these as a symbol. Talk to a mason if you can one of ur grandpas colleges perhaps sorry i couldnt be more helpful
