    Here is my take on it. Translation is for YOUR numbering.
    Veteran's full name is Lyshinsky Genady Nikodimovich - Люшинский Геннадий Никодимович.
    Lightly wounded 24/Aug/1942 near Stalingrad. He was awarded CSM medal in June 1945.

    1. This is registration visa. The exact name is difficult to decipher.
    "Registered.
    The head of district MGB. Signature
    7 Detached Signals Brigade of RGK (RGK = Reserve of the Supreme Command)
    Captain /Youvzhenko/
    28/XI-1946".
    2. Surname: "Lyshinsky" (Люшинский)
    3. Name and middle name: "Genady Nik.".
    4. Rank: "Private"
    5. Unit: "233 AZS Reserve Rifle Regiment." this unit belonget to 57 Army.
    6. Unit (battalion, company): "6 Company"
    7. Personal signature: bla-bla-bla
    8. Company Comander signature: "Lt. Sizov"
    9. this is the date Company Comander signed: "31.07.1944"
    10. Military speciality: "134" (This is the code for servicemen limited fit for active duty and who undergone some kind of specialized education. It fits well to his service unit beign "reserve rifle regiment").
    11. General education level: "5 years of school". (i.e. elementary)
    12. Ethnicity: "Belorussian"
    13. Birth year: "1923"
    14. Year of military draft: "23.05.1942"
    15. Drafted by which military commissariat: "Shaltinsky regional military commissariat". (Шалтинский РВК ?)
