    Hi Tony,
I can't really read German so maybe I'm wrong here but
I don't see anything that indicates the guy was a physician.

I'm sure someone who actually knows will help you out.

"Under-physician" or "sub-physician" is the literal meaning of "Unterarzt", although a better translation would be "junior physician".

The rank of Unterarzt was an SNCO rank equivalent to Feldwebel that was held by military physicians who, having completed their course of studies and successfully passed their physician's exams, were now serving with a military unit.

(The next-higher rank was Assistenzarzt, which was equivalent to a 2nd Lieutenant. After a minimum of three months service as an Unterarzt, the candidate could be submitted for promotion to this rank.)

The man in question was an Unterarzt serving with the III. Battalion of the 6th Rhenish Infantry Regiment No. 68.
The document is signed by a Major acting on behalf of the Regimental Commander. ["I.A.d.R.K." = "Im Auftrag des Regimentskommandeurs".] The award was made by authority of the Commanding General of the VIII. Army Corps in the name of the German Emperor and Prussian King.

    I see now
I should not have relied on the very first definition of Unterarzt
that I came across on the WEB
