That's Infanterie-Geschütz-Ersatz-Kompanie (motorisiert) 128- a part of Infanterie-Ersatz-Regiment 128.

Sure thing. And it doesn't belong to any Division- it's a replacement unit. On paper these would supply a few specific Regimenter, but wartime needs sent men all over, not to mention that transfer was common as well. You can look on the Lexikon der Wehrmacht site posted about in the sticky at the beginning of this section to find IER 128 and research what units it might have supplied to perhaps give you some idea of the soldier's possible history, but that's about the best you can get. If you know where the disc was found, that can sometimes help narrow it down.
