It is a soviet passport not a polish one.
But the Soviets also issued them to former Polish citizens after 1939, during 1940.
I am not sure if it was a Ukrainian Soviet passport or one issued in FORMER eastern Poland...
A Soviet passport issued in 1940 after the Soviet occupation of eastern Poland in 1939.
It was issued to the resident of city of Borislav in Drogobych oblast (region) - currently in Lvov region in Ukraine.

The last page refers to the residence registration dates/terms.
