It says:
"Polish Duty (Offficial) Passport Seria W-1413 issued through Ministry of Foreign Affairs in Warsaw on 4/9/1946.
Hospital issued certificate from "Ramat Marpe" hospital in Ramat Gan, Palestine on 30/9/1947 showing birth of a son Zygmunt.
This passport is valid for one time crossing into Poland within the valid time frame through allowed border crossing in ------
Tel Aviv 18/10/1947 there is a handwritten note : "Crossed by the office"
This passport loses its validity after the owner has crossed into Poland and the passport cannot be extended.

Would this indicate she had a previous service/pfficial passport from 1946?
Why then she was issued this passport then?

My guess is that she was one of the People who escaped from soviet paradise along with Anders Army and most likely had no official papers of any kind and after the War she wanted to go back to Poland.
Yes, but it might be she was used a service passport instead earlier, then being replaced by this 1947 issue for returning back to Poland?
With only that page of Passport showing and no additional info we can only guess.
