Stansilaw Jankowski's badge number is correct. Decorations BKZzM, KPMC, MW, 1Sz
Badge 569 belongs to st.panc. Edward Stasiewicz
Badge 069 belongs to ppor. Alfred Kornreich
There is no Piatrakowski that I can find listed in the regiment.

I have just looked through my copy of "Walczace Skorpiony" Historia Pulku 4-go Pancernego w Fotografi, Londyn 1975, ( Photographic History of the 4th Armoured Regiment, published in London in 1975)
I have found the following information with regards to the above personnel.
St.Panc. Stanislaw Jankowski, Born in 1918, Army Number: 1918/84/III, 2nd Squadron, 4th Armoured Regiment, awarded the Monte Cassino Cross Numbered: 29202, he is also listed on the List of Recipients of the Virtuti Militari with the rank of Kapral (Corporal).
St.Panc. Edward Stasiewicz, Born in 1916, Army Number: 1916/100/III, 3rd Squadron, 4th Armoured Regiment, he was awarded the Monte Cassino Cross Numbered : 29557, he is also listed as a recipient of the Krzyz Walecznych (Cross of Valour).
With regards to Ppor. Alfred Korniech, I found him in the 3rd Squadron awarded the Krzyz Walecznych x 2 (Cross of Valour and Bar), also received the Wloski Croce Al Valore Militare, (italian War Cross for Military Valour), he also was awarded the Polish wound star (Ranny) wounded once in combat.
When it came to finding him on the Monte Cassino Cross recipients list Ppor Alfred Korniech was no where to be found in the 2nd Armoured Brigade list, then I had a thought what if he was promoted from the Ranks and Bingo I found him in the 4th Armoured Regiment.
When fighting at Monte Cassino he was a Kapral (Corporal) Pdch. (Officer Cadet), he was Born in 1919, his Army Number: 1919/360/III and was awarded the Monte Cassino Cross Numbered: 29246.
I also had a quick look for Plutowny (Lance Sergeant) Piatrakowski, but with No luck, I will have another look for him later and if I find anything, I will let you know.

I have also found the following information on Ppor Alfred Konreich:
Date of Birth: 27th July 1919
Place of Birth: Krakow
Parents: Father: Matias Kornreich, Born: 1886 in Malinowka, Mother Ernestyna Konreich (nee Abeles), Born 26th March 1890 in Krakow.
Alfred Konreich also had a Brother Ludwik born 7th September 1915 in Krakow.
Alfred Konreich attended the 2nd High School of King Jan Sobieski in Krakow and graduated in 1937.
