I have had time to look through the Register and I have found Kapral Zygmunt GACZYŃSKI, 1901-17-III, he did serve with the 1 Kompania Zaopatrywania (1st Supply Company), 3DSK awarded the Monte Cassino Cross Numbered : 4589.

For the first time I saw it on auction from the UK. The seller as I remember was also from the UK. Please take a look at the photo of the three decorations.Look at the background of the photo.
