A pinhead is a person born with a condition known as microcephaly. It is a neurological disorder and is characterized by a smaller than average head.
So let us be PC, and say, the dude had a small head...

Hahaha point well taken, well from the pictures Ive seen of them being worn im not sure these helmets were meant to fit down over the head as much as just rest on top, I think the style of them is just small in general. 54 to 58 1/2 are the only sizes Ive seen.
