Short description of personal combat feat or accomplishment
During offensive operations 13.7.44-22.7.44 he displayed bravery and courage. With his platoon assigned to the infantry formations, the direct fire of his platoon destroyed a platoon of enemy infantry, one machinegun, and scattered as many as two platoons of the enemy.
While occupying Rava Russkaya on 20.7.44 his platoon entered alongside the infantry.
He is deserving of the Order of the Patriotic War II Class for displayed combat bravery and heroism.

The first documents says that Poletayev received military booklet (voyenny bilet) nr. GI-062092 on 9 Dec 1968 by the military commissariat of the Lenin Raion, city of Chisinau. He was then living on 6 Kavkazskaya Street, apartment 1. Signed by the chief of the 3rd section, Major Kostenko, on 9 Dec 1968.
The second doc is a "Check card" (kontrolnaya kartochka) for the military booklet and lists name, personal number, rank, and year and place of birth.
The last two scans are his wartime service record, the predecessor of the one with photo you showed earlier.
