I see some simularities in the example Ned posted and the one that started the thread, both don't have space between 'IM' and 'OSTEN'. It reads like one word... They both have a weird looking '2' in 1941/42.

Hello,
All I know about the Spanish ost medal is,
They came out in 1950's for veterans of the blue division and for German soldiers who lived in Spain to wear. At the time Spain was in a fascist government so it was not illegal to show a swastika on your award.
Many fake items however came from Spain in the 1970'a and 80's.
There has been a book wrote about spanish blue division awards by a member of Wehrmacht awards ( I think) so it might be best to contact him there for a good opinion on your award.
