Actually that was meant to be the middle finger.
The first one I have seen, is there any picture of the other side. The eagle looks like the SA type.

The hand is in a formal oath-taking gesture, so between that, the steel helmet and the Party eagle, I would say this is about the armed forces' allegiance to Hitler and the Nazi state.
It looks somewhat familiar; I think it is at least based on some period artwork, but I can't quite put my finger on it, so to speak.
The detail is rather poor, i am unsure why a medal would be made for swearing the oath.

I knew I had seen the design before, and I finally found it. The hand/helmet/eagle design was used on the event badges for the 1936 Tag der Wehrmacht [Day of the Armed Forces]:
