Wow! Thought you had found a .22 pen-gun there for a second! But it looks very much like an old casing for a medical thermometer. My grandmother who was a nurse had a thermometer in a similar pen casing. Hope it helps.
Now that you mention it, a thermometer case sound best to me.........!

Beet me too it......."Small medical mercury thermometer carrier".
I have something very similar for a modern day thermometer I used to use in the field back in my power plant instrumentation days.
