It is an optical device inspection record. The bottom lines read, "Administration of Press and Publication of the Armed Forces, 1963".

I have no idea but that isn't my father-in-law. He left in the late 60s to escape the tyranny and began a new life in the UK. He lost most of his family in the second gulf war.
