I think Perth was three times attacked by air in the Mediterranean in 1941 over three different days.
I will go with south of Milo in the Med. 22rd May 1941.
If I am wrong I won't have a crack at the other date and place.

Attacked by air off Malta in company with HMS Illustrious 16th jan 1941 as well as 21-22 May off Milo/Kaso strait.
