I'm concerned that it might be cast. It looks like it has a couple of pits on the back, and a hairline on the front. Am I being paranoid?
Hardly any detail on the Czars head, but it could just be heavily worn!...

Hi Paul, Imperial Russian is not my thing. But my concern is this is a casting too. I am looking at the lettering to the reverse and it looks poor.

The suspension ring looks to have been soldered on to the medal disc, it should be part of the stamping.
Some Russian/Soviet medals tend to be quite "soft" in construction. Suspension rings can wear in contact with the ribbon ring (the top of the suspension ring) on these hence a later replacement . Seen a few like this over the years. Food for thought.
