I can guess that one is cavalry and one is medical but I don't know about the plain one. Any Ideas?

Hello, thanks for posting your nice buckles. you are right about the cavalry and medical, the plain ones are usually associated with civil militias particularly during the civil war on both sides of the conflict. Also the lack of crowns could indicate the Republic era, post 1931. In fact the Condor legion also used plain buckles but without provenance there is no way to prove that association.

    Cheers, Al

I have seen a few cut out buckles like that from pre-republic days of Alfonso XIII usually denoting infantry or artillery regiments. Is that what they are ?

    Yes.
Buckles like this were used in the Spanish American War by Spanish Army troops.
