It's a sterile wound dressing with an attached bandage, called a Verbandpäckchen in German.
"Faden abstreifen" is simply an instruction, meaning "remove thread".
The thread it refers to was wrapped around the outside (one horizontal and one vertical pass, crossing on the obverse and reverse sides) and has already been removed somewhere down the line.

Looking at one in my collection now. The thread is white and very thin. It is tied at the back. It is marked to indicate that it was sterilized at high temperature. The wording changes on these but something like Im Dampf 120 o entkeimt. NH
