It translates as "Please breathe on mirror"; it's not an idiom but a simple instruction.
No doubt that this was once a pocket mirror that has the actual mirror missing from the protective cover.
As to why one should breathe on it, I don't really know. The best guess is probably to see whether someone was still breathing when checking their vital signs. (At that time this method was still taught.)
