Ok, I did a little research myself and I've concluded that this table medal was awarded to (?) the "10 Sudecka Dywizja Pancerna im. Bohaterów Armii Radzieckiej" (previously: 10 Dywizja Piechoty, which took part of the fighting in western Poland and Eastern Germany in 1945.) Can somebody tell me where the "14.1.1945" is referring to? And why this medal was made in the first place?

I am sure the medal was many years after the WW II.
During communist period this form of award was very popular - to commemoratite any event, for merits, etc.
