German federal election ballot , March 1933

I purchased this a while back and until a recent find in a book, I completely forgot about having it.
I was told when I bought it, that this was a election ballot from the March 1933 German federal election.
And that choice#2 represented the NSDAP vote.
I know there were many elections, in many areas and districts. So exactly what and where this is from, I don't know.
Any thoughts and knowledge in this field is appreciated.

I think it s a parlimentary voting form for the district of Franconia. Yes the second option is for the NSDAP, and also there are representatives for the communist party #3 on the ticket as well (wonder what happened to those fellas). Never heard of the district of Franconia before, so I Googled it and learned this was the district that Nuremburg was in.
