Replacement wehrmacht-fuhrerschein

Hi Chaps, I'm after some help with the translation of this replacement drivers licence I recently picked up. I can pick his name out (Josef Follmar, I think?) & the odd word; but after that my poor German has reached it's limit.
This is the first time I've seen this style, as opposed to the cloth version. I suppose this one was meant to have a photo; but being issued on 15/02/45 the owner didn't get 'round to it.
I like that it was printed in occupied Paris.

(25.9.1940-15.2.1941) 3. Kranken-Kraftwagen-Zug 66.

He was licenced for Klasse 3 which was for Cars.
