I agree with Paul, largely because it looks utterly new. It is certainly unused and in excellent condition for a form that is at least 68 years old. I can't imagine why in a normal situation anyone would go to the effort to counterfeit a form of such little worth other than that it has SS on it, but in this field anything goes. I vote No on it.
