ID Disc Decipher Please

I wonder if anyone can decipher this from an ID disc. I think this soldier may have served with 1st Anti-Aircraft Regiment (1 pułk artylerii przeciwlotniczej)

PPOR ART AP GAJEWSKI 1919 WITOLD RZ KAT - Name - Witold Gajewski? DoB 1919 Roman Catholic?

PPOR means Podporucznik which is 2nd Lieutenant.

A.P = Armia Polska = Polish Army - standard on Polish tags.

I think it is unusual to have rank and unit included on a tag, so I am not sure what the ART means here. It is usually the abbreviation for Artylerii. Also the soldier's serial number is missing. I am no expert on tags though, just seen 30 or so and none with quite the data like this one.
