It's HMS Hogue, a Bacchante Class Cruiser. She was sunk along with her sister ships Aboukir and Cressy by the submarine U-9 on the 22nd of September 1914 between 0:620 and 07:55. Details of the action can be found here:

They were known as the live bait squadron on account of the vessel's obsolescence. I have the original service record and some sketches done by a signaller who survived the sinking of Cressy.
