The cap and insignia look good. I cannot comment on the hand written tag.

The garrison cap and emblem look good.

The note looks like PLT 1134 =Platoon 1134 . I'm not sure about the 81.
    Pvt Murphy = Private Murphy

Considering Murphy is a common last name and there is no first name, Tough one.
But, If you could find the class that he graduated in (Year) would be the first step.

One thing though, Is that Marine's mark there gear with an Ink stamp. We put our name on all of our uniforms.

Having it hand written on a piece of paper that is stapled to it................. Another Tough one.

Zombie Response- The 69 in the DSA may just refer to the year of approval. I'd have to look at my '84 issue piss cutter.
The 81 on the name tag might be his Recruit Number in the platoon.

Definitely a boot camp cover as iac said, the PLT is boot camp platoon and 81 is the number alphabetically the guy fell almost the recruits in the plt.

Also as Phil said it’s odd that it’s written on paper and not ink stamped.

Update- Couldn't find my covers but the blouse & trousers are marked 82 & 83. So I would have to say the 69 is the Manufactured Year.

DSA 69 is the year of manufacturing , for Nam items.

Metz also made these! 69 is the awarded contract year.
