I don`t know much about your document other than it is a reference after leaving a job.
The text basically says the following
Reference to Prudinskiy ( persons name). He worked as a Doctor from 26.01.1945 until 14 06.1945. He was very conciencious to his work and resposibilities
It is then signed by ( unfortunately i cant read the name properly) by the Director of Personnel Department NKVD USSSR ( UKRAINE)
Then signed by the head doctor

Unfortunately not Niel. The ink stamps are too blurred to read properley so i can“t tell you of any town or place etc. Being signed by the director of NKVD Personnel i presume it would have been a Military doctor

I would say this certificate was issued in NKVD prison, and signed by chief of the prison.
May I ask why you think it was issued at a prison, for who and why?

Sure. A title of a person who signed it sounds like--- Chief of ITK NKVD UzSSR. A key point is ITK ["I" - correctional, "T" - labour, and "K" - facility].
I do not thik a person who recieved this certificate was a prisoner, since before his name is indicated letter "t". It means TOVARISH. In case if he was prisoner there should be "gr." It means GRAZHDANIN. So TOVARISH cannot be prisoner.
So I think this person was a usual doctor in the prison hospital.
