The owner might have been fortunate not to lose this one as the top pair of holes are very close to the edge of the metal.

I think the blank itself was regular machine made.
So it doesn't have to be a replacement.

Sometimes the punching was a bit decentralized.
Here an extreme example of an unused tag found by a colleague. It reminds me always on Space Invaders game.
