I was looking at the Glory Orders on Wikipedia and apparently, some were awarded 4, and one pereon, 5... Apparently, due to presidium error.

Yes, that hapened. Even more often a double OG III was awarded which would later be upgraded to an OG II and the same happened with double OG II which were upgraded to OG I.
