Hello,
I won't comment on the cap, not really my area of expertise.
Concerning the Dienstbluse, it's rather nice, most probably privately purchased around 1935-36.
I have already seen Officer collar tabs sewn this way, although it was not really the norm.
I was just wondering if this Officer used these collar tabs or if they're a later addition as they're not meant for Sanitäts personnel, but for Nachschubtruppen (transport). As you can see, the blue on the collar tabs is much lighter than on the shoulderboards (this blue is the correct one for medical troops).
I do own an Officer set of insignia for medical troops from the same Stabsartz (Captain), collar tabs are dark blue, while shoulderstraps are also lighter.
I love the cut of the back of the tunic, such a reminiscence of the imperial time...
Concerning the Hoheitsabzeichen, a better photo would help.

To second what JPhilip says, although usually hand-stitched for officers, I have even seen machine-stitched Generals’ collar patches.
It is a very nice looking tunic.
