Seeking information on my wife’s Dziadek military service

Firstly, thank you for allowing me to join. I am an ex-Australian Army Warrant Officer with a passion for military history. My wife (who is Polish) recently lost her Dziadek, which affected her greatly, due to their close bond. I am seeking information on his military service, as I would like to purchase the uniform and beret that he would have worn. The information I have on him is he;

Former Service and History: Voluntarily enlisted with the Polish Forces under British command.
Served in Italy and the United Kingdom until finally discharged on
repatriation to Poland,
Posted to the 14 Wilenaks Rifle Battalion of the 5 Kresowa
Infantry Division, 2nd Polish Corps on 28.7.1945.
Seconded to a Secondary School
Coupee with the 5 Kresowa Infantry Division at Modena, Italy fron 20,11,1945 to
13,5.1946.
Joined the 5 Krenova Infoptry Divedon Secondary Behool on 11.6.1946.

I wondering what uniform, badges etc he may of worn and what medals he was awarded or entitled to, as I would love to purchase them as a surprise for her. Any assistance would be greatly appreciated.
