It is really almost impossible to say anything in particular, since the silhouette of the pins on your great-grandfather's lapel can hardly be distinguished.
The only thing I would dare to almost assure is that what is more to the right of the image can be a buttonhole ribbon “Knopflochdekoration”.

The silver looking award is shaped similar (thin oval) to some WW1 flying badges or tank memorial badge. Although overall size is I think smaller than original. This is just a best /worse guess
