It's Freikorps-related and not Russian per se, although there is a connection:
The design of this item is that of the Medaille der Eisernen Division [= "Medal of the Iron Division"] which was awarded by the Verein ehemaliger Angehöriger der Eisernen Division [= "Association of Former Members of the Iron Division"] on behalf of Prince Awaloff, Commander-In-Chief of the Russian Volunteer Western Army, to members of the Eiserne Division for participation in the fighting in the Baltic.
Lacking any means of suspension, this one appears not to be the actual medal as worn from a ribbon, though. Some details of the skull appear to differ, too. Sorry; I cannot comment on exact purpose or authenticity of the item.
