I know of entyre collections in private hands coming from the museum, some pieces even with the museum inventory tags on it.

they have a complex system in command. a civilian director, a military director. Civilian personel, military personel. 2 languages...
So, yes, shit happens. One director was replaced since he could not get an exposition ready in 2014 for the 100year WW1...
(He had 100years time to achieve that).
They don't work efficiëntly and it takes year to change something.
