The Rudolf Souval ISA's can be quite puzzling due to this maker continuing production well into the post war period.
In regards to your questions:
1. Yes, my opinion is this one is an accepted wartime R. Souval variant.
2. Wartime
3. These are what is referred to as die flaws
4. I will post some of my examples.
First, some recognized wartime variants with like hardware set up as yours displays.
Also, the second example is from an LDO Store Hoarde find some years back.

Here is the Rudolf Souval Large Font variant. This one is also considered a Wartime example.
Post War Variants.
Notable attributes of the Post War Examples.
The missing wreath leaf between the Kar98k and the sling and the tell-tale Claw style catch that is indicative of these Post War Souval examples.
Note, the bayonet die flaw is also present on these which only adds to the confusion.
First is a variant with the Swazy still intact.
Secondly, is a denazified Post War example.

To complete this thread on the R. Souval ISA variant, here are some known fakes of this example to assist when researching this maker.
First, here is a copy of the wartime Rudolf Souval.
Key attributes that stand out for this one are the incorrect head sculpt and the sloppy reverse.
Next is the Post War copy.
Whats funny about this one, its a copy of the original Post War Copy.
Note, this one has the same incorrect, pigeon like head sculpt of the previous fake, missing wreath leaf between the Kar98k and the sling along with a reverse pattern that is more close to the mm "A" Assmann solid ISA's than anything Rudolf Souval ever produced.

I don't quite understand what you wrote about the wartime fake. Were fakes made during the war, but why?
Or did you mean post-war counterfeiting of a badge on wartime equipment?
