Of course, it is not frequent to find it like this with the EK2 1914. It´s the first one I have seen.
in case I may have missed it, what is the S shaped hook that is on it? the button hole one.
It was the ring from which the missing miniature medal hung. When it opens and deforms, instead of being a ring it looks like a hook or a letter "S"

The Karl and Olga Medal was established at the initiative of Queen Olga of Württemberg, with the consent of her husband Karl, for the celebration of the Silver Jubilee (25 years) of the king's accession to the throne, on June 16, 1889.
The award was given to men and women who have distinguished themselves by providing honorable service to the German Red Cross in caring for the wounded and sick (especially soldiers).
The award ceremony was held at the queen's request, and always by the King of Württemberg.
The medal was awarded in silver and bronze versions, and presented on the front the embossed effigies of the reigning couple, haloed with a red enamel cross and equal arms.
Around it, forming a circumference, a band with the phrase: KARL UND OLGA KÖNIG UND KÖNIGIN VON WÜRTTEMBERG (KARL AND OLGA KING AND QUEEN OF WÜRTTEMBERG) On the back was an inscription in 4 lines: FÜR VERDIENSTE UM DAS ROTE KREUZ (FOR MERITS TOWARDS THE RED CROSS) and below, a 6-pointed star. On the band that surrounds everything are the words: VON DER PROTEKTORIN DES WÜRTT. SANITÄTSVEREINS VOM ROTEN KREUZ. (UNDER THE PROTECTION OF THE WURTHBURG RED CROSS HEALTH ASSOCIATION).
The medal was engraved by Württemberg court medalist Karl Schwenzer.
In 1916 the award was discontinued in favor of the Charlotte Cross.
The production number is given as approx. 1000.
