Believe it or not, quality control "slips" were not unheard of in the DDR despite the public posturing about how great everything was in the socialist utopia! That is an extreme example though and collectible in the same way that postage stamps without perforations or mis-struck coins tend to be. It should not have escaped the melting pot!!
As an aside, if the worker was bladdered I doubt it was Russian vodka. Even the Soviets (other ranks and junior officers anyway) had problems getting it. My colleagues and I were able to buy Stolichnaya (Capital) vodka in Natashas Russian Giftshop in E. Berlin and trade it with Soviet troops for badges and stuff. They used to go nuts for it "Stoli" was uncommon in the UK back then but now I think most pubs and bars sell it.
It's hard to imagine how that buckle ended up like that as I believe they were pressed into the 3D shape from a flat blank stamped from sheet stock Perhaps the black was not correctly seated in the die of the press?
