Need help with translation into Cyrillic please.

I recently received this award booklet to Dmitriy Zacharovich Shylgin who received two Bravery medals, an Order of Glory III, an Order of Red Star and a medal for War Merit. I want to check him up at the Pamyat Narod website but I can't type Cyrillic on my laptop which makes it impossible to check. Does any of you want to write his name in Cyrillic here please so I can continue my research?
