This cap is dubious at best, but I will only comment on the Totenkopf. While this type of skull does exist I believe the one the cap is a modern fake. Look at these original skulls from my collection and compare, then look at this Japanese modern production skull.

Compare to these images from an esteemed colleague for a proper cap of this era.
